BBL in Tunisia: Buttock Fat Transfer Explained Plainly

The BBL is one of the most delicate procedures in cosmetic surgery. Here, plainly, is how it is carried out, what makes it safe or dangerous, and who it suits.

The BBL (Brazilian butt lift) is a buttock fat transfer: fat is harvested by liposuction from your own body, purified and then injected into the buttocks to increase their volume and reshape them. In Tunisia as elsewhere, it is a demanding procedure, whose safety rests almost entirely on one technical point: the fat must be injected only into the subcutaneous plane, never into the muscle.

It is not an ordinary procedure or a minor one. It is recognised as one of the riskiest in cosmetic surgery when the technique is not rigorous. It requires a prior consultation and a full medical examination, and the quote is only provided afterwards. No result can be guaranteed.

This article explains plainly how the BBL works, where its danger comes from, who can be considered and what recovery looks like.

How a BBL is carried out

The operation is carried out under general anaesthesia. It has three stages: harvesting fat by liposuction (abdomen, flanks, back, thighs depending on where you have reserves), processing this fat, then re-injecting it in small amounts into the buttocks, in several planes and along several tracks to distribute it.

  • Pre-operative marking, standing, to draw the donor and injection areas.
  • Liposuction of the donor areas, through small incisions of a few millimetres.
  • Preparation of the fat: separating out fluids, blood and damaged cells.
  • Injection through micro-cannulas, gradually and evenly distributed.
  • Dressings, compression garment and monitoring in clinic.

Why the injection plane is a safety issue

The buttocks are crossed by large veins, located in and beneath the gluteus maximus muscle. When a cannula punctures a vein or fat is injected into the muscle, the fat can pass into the circulation and reach the lungs. This is called fat embolism. It is a rare event when the technique is mastered, but sudden and often fatal when it occurs.

The safeguard comes down to one rule: fat is deposited exclusively in the subcutaneous layer, between the skin and the muscle, with a cannula whose depth and orientation are constantly monitored. Some surgeons use real-time ultrasound to visualise the cannula and check that it does not enter the muscle. Others keep to a strict subcutaneous technique, with limited volumes and a flexible cannula. The principle remains the same: never fat in the muscle.

One consequence must also be accepted: in the subcutaneous plane, you cannot inject as much volume as in some deeper planes. A surgeon who promises spectacular volume in a single session should give you pause. A less dramatic result, achieved safely, is better than the reverse. For greater volume, a buttock implant may be discussed.

Fat reabsorption: on average 30 to 40% is lost

Not all the injected fat survives. Some of it is reabsorbed in the weeks and months that follow, because not all fat cells obtain an adequate blood supply. On average, it is estimated that 30 to 40% of the injected volume disappears, with wide variation from one person to another. The volume you have at three months, then at six months, is what remains.

This loss is anticipated: more is injected than the target volume, without overdoing it. It cannot be predicted for an individual. A further session is sometimes considered to refine the result, but never automatically. The fat that survives is, however, lasting, because it becomes living tissue. It does, though, follow changes in your weight.

Recovery: sitting and lying positions, compression

Recovery from a BBL differs from that of other liposuction, because the buttocks must not be crushed. Prolonged pressure can reduce the blood supply to freshly transferred fat cells and decrease their survival.

  • Sitting: to be avoided or strictly limited for about two to three weeks, according to the surgeon's instructions. A special cushion may be prescribed.
  • Sleep: on your stomach or side for several weeks, never on your back at first.
  • Compression garment: worn day and night for several weeks for the liposuction areas, with suitable openings so as not to compress the buttocks.
  • Activity: light walking from the first few days; exercise postponed for several weeks.
  • Oedema and bruising: significant at first, they subside over several weeks.

For a stay in Tunisia, allow about 7 to 8 days with one night in clinic, often longer if other procedures are combined. The return flight is permitted by the surgeon only, and prolonged sitting remains a point to plan for.

The donor areas: a result for the silhouette too

One advantage of the BBL is that liposuction of the donor areas slims the waist, flanks or abdomen, which accentuates the contrast with the buttocks. But these areas can show irregularities, waviness or loose skin if the skin lacks elasticity. The result on the silhouette therefore depends on the quality of the skin as much as on the technique.

Who is not a candidate for a BBL

The BBL is not for everyone. An honest consultation sometimes leads to deciding against it, or to proposing another option.

  • Insufficient fat reserves: with no fat to harvest, there is nothing to transfer. A very slim person is generally not a candidate.
  • Unstable weight or a pregnancy planned soon: weight changes alter the result.
  • Active smoking: it impairs blood supply and healing. Stopping is required for several weeks before and after.
  • Clotting problems, a history of thrombosis or severe heart or lung disease.
  • Unrealistic expectations of volume or shape.

The risks of a BBL, without downplaying them

Apart from the fat embolism already described, the BBL carries the risks of any liposuction and of any surgery under general anaesthesia: haematoma, infection, venous thrombosis, delayed healing, contour irregularities, asymmetry, fat necrosis with cysts or lumps, and loss of sensation. They are explained at the consultation and set out in the informed consent form.

To limit these risks, the surgeon must have specific training, work in a clinic equipped with an anaesthetist present throughout the operation and with resuscitation facilities, and respect reasonable harvesting and injection volumes. Do not look for the cheapest offer: in the event of a complication, having a team nearby ready to act matters more than the price.

BBL in Tunisia: the process

The extent of the liposuction and the length of time in theatre are decided after the medical examination, at the consultation.

The typical process includes a video consultation, a trip of a few days, an examination, a pre-operative work-up, an anaesthetic consultation, the operation, then check-ups before the flight home. Follow-up then continues remotely at agreed intervals.

Frequently asked questions

Is the BBL dangerous?

It is one of the most delicate procedures in cosmetic surgery. The danger comes from injecting fat into the gluteal muscle, which can cause a fat embolism. It is reduced by strictly subcutaneous injection, possibly guided by ultrasound, and by a properly equipped medical environment.

How much fat remains after a BBL?

On average, 30 to 40% of the injected fat is reabsorbed, with wide individual variation. The stable volume is assessed at around six months.

How long should I avoid sitting after a BBL?

Generally two to three weeks, with sitting avoided or limited so as not to crush the transferred fat. The surgeon will specify the duration and may prescribe a suitable cushion.

Who cannot have a BBL?

People with too little fat to harvest, an unstable weight, active smoking, clotting disorders or a history of thrombosis are generally not candidates. The examination settles the question.
